Many clinical ethics and legal guidelines for therapy tend to be similar across disciplines. You may use the paperwork forms if you are a clinical social worker, mental health counselor, marriage and family therapist, or psychologist. The terms “clinician” and “client” are used in the documentation forms to be inclusive. However, check your state licensing board for any discipline-specific provisions that should be added.

  • I can’t find certain paperwork forms. Are you going to add more?

    You may be looking for forms like a superbill template or a biopsychosocial assessment that is not included as a paperwork product. Forms that you wouldn’t need to pay for because they’re easily accessible in your EHR system are not included for purchase. You can contact me for help with finding forms that are not listed for purchase. However, additional forms and templates will continue to be added. If you have a form you would like to be added, I welcome your feedback!

  • Don’t some paperwork forms come with EHR systems?

    Yes, some paperwork forms come with EHR systems and can be readily used if they’re for templates like superbills or assessments. However, documentation forms that include important ethical and legal provisions need to be more comprehensive. For example, you may find that a statement of informed consent can be found in your EHR system but does not include specifics to telehealth, minors, couples, and families, international clients, etc., and unfortunately, some EHR systems may not come with documentation forms for therapists at all.

  • What forms do I need to send to clients before their first session?

    As a mental health therapist, you are required to present clients with a statement of informed consent or counseling agreement and HIPAA privacy policies at the minimum.

  • Can the forms be used for a group practice?

    Yes! The paperwork forms are written in the second person (e.g. “your clinician”) and can be used for both solo and group private practices. Please contact me for group licensing pricing.

  • Can I buy the paperwork forms and share them?
